Attention Parents … The summer months can be a busy time for your children as the school year ends and summertime begins. Below are examples of some of the types of forms or documents that need to be notarized: -Permission Slips Do you need a permission slip signed by you to allow your children to participate in extracurricular activities and to hold the school (or other entity) harmless for anything that happens during those activities such as, field trips to zoos, amusement parks, historical sites, and picnic areas, as well as forms to grant permission to a swimming instructor to give your children swimming lessons. These permission slips require notarization. -Hold Harmless Agreements Are your children going to summer camp? If so, you may need this type of agreement to be signed and notarized to not hold certain the camp or instructors liable for accidents or injuries to children. -Medical Treatment Consent Forms Do you need a medical treatment consent forms notarized? These forms allow another adult to authorize medical treatment for their child in case of an accident. -Minor Traveling with One Parent Consent Forms Do you have minor children that need consent from you to allow them to travel with one parent or another adult who is not the child’s parent? These forms require notarization.
